Background: The new human coronavirus that leads to COVID-19 has spread rapidly around the world and has a high degree of lethality. In more severe cases, patients remain hospitalized for several days under treatment of the health team. Thus, it is important to develop and use technologies with the aim to strengthen conventional therapy by encouraging movement, physical activity, and improving cardiorespiratory fitness for patients. In this sense, therapies for exposure to virtual reality are promising and have been shown to (...) be an adequate and equivalent alternative to conventional exercise programs.Aim: This is a study protocol with the aim of comparing the conventional physical therapy intervention with the use of a non-immersive VR software during COVID-19 hospitalization.Methods: Fifty patients hospitalized with confirmed diagnosis of COVID-19 will be divided in two groups under physiotherapy treatment using conventional or VR intervention: Group A: participants with COVID-19 will start the first day of the protocol with VR tasks in the morning and then in the second period, in the afternoon, will perform the conventional exercises and Group B: participants with COVID-19 will start the first day with conventional exercises in the morning and in the second period, in the afternoon, will perform activity with VR. All participants will be evaluated with different motor and physiologic scales before and after the treatment to measure improvements.Conclusion: Considering the importance of benefits from physical activity during hospitalization, VR software shows promise as a potential mechanism for improving physical activity. O cerne desse artigo remete à instauração de um dispositivo de acolhimento e escuta de orientação psicanalítica numa instituição educacional pública de nível superior. Sustentamos a práxis em uma experiência político-clínica de acolhimento na Universidade, salientando sua função terapêutica como forma de resistência coletiva a um modus operandi absolutamente individualista. Cada investida neoliberal em nossa sociedade é capaz de produzir engendramentos subjetivos da pulsão de morte, resultando em sofrimento e adoecimento na medida em que o sujeito é abandonado à própria (...) sorte, desamparado, num tecido social que se esgarça. Para fazer face a tamanho desalento, recorremos a um projeto de extensão, delineado enquanto um dispositivo de escuta psicanalítica gratuita, no seio da Universidade Pública, oferecido à comunidade acadêmica, a educadores e, principalmente, a jovens estudantes em estado de intenso sofrimento psíquico. O presente texto propõe-se a refletir, no rastro de Arendt, sobre o declínio da política na modernidade diante de sua redução à administração da vida e das necessidades vitais. Para tanto, discutiremos a “moderna alienação do mundo” fomentada pela ascensão do trabalho e da vida, e refletiremos sobre a redução da política à administração da vida diante do advento do social e da vitória do animal laborans. Pressupõe-se que, examinarmos tais categorias nos permite observar o ocaso da política na era (...) moderna e a sua transformação em biopolítica, e, ademais, como esta categoria segue na contramão da noção política da autora. Este trabalho tem por objetivo apresentar algumas considerações sobre a noção da experiência no âmbito da filosofia da educação. Na transposição dos ideais formativos iluminista e romântico para os sistemas escolares ocorre a marginalização da experiência, a centralidade das noções modelares hegemônicas (a cientifica-especializada e a crítica-reflexiva) e o problema da redução da experiência ao empírico. Na contemporaneidade, indicamos caminhos alternativos para a retomada da experiência no campo de estudos da filosofia da educação.

Desafios e dilemas da condição de trabalho de professores iniciantes no magistério público no DF Resumo: Este trabalho se origina nas preocupações voltadas às condições de trabalho de professores que iniciam na docência e tem por objetivo estudar a condição de trabalho do professor no início da carreira, buscando entender os desafios e dilemas enfrentados, no intuito de contribuir com a análise da realidade da atividade docente. O foco é o professor iniciante na Secretaria de Educação do Distrito Federal. Os (...) resultados são obtidos a partir da análise documental em legislações e publicações no Diário Oficial do DF, e na aplicação de questionários e entrevistas. Os dados revelam um quantitativo expressivo de desistentes na carreira, compreendido em um terço dos professores que são aprovados no concurso público, anunciando questões objetivas ligadas as condições de trabalho, nos levando em questionar a necessidade de políticas de formação pensadas a partir da realidade do trabalho docente. Chamamos atenção para a omissão de políticas públicas diante do caso, como um fator condicionador para intensificar ainda mais as dificuldades enfrentadas nesta etapa da carreira. Se espera contribuir para a produção de conhecimento na área de formação de professores no que se refere aos dilemas vividos pelo professor iniciante, visando fundamentar propostas sobre acompanhamento profissional dos docentes iniciantes na carreira. No período pandêmico ocasionado pelo Novo Coronavírus, surgiram muitas Fake News nas redes sociais digitais, gerando desinformação e prejudicando a Administração Pública na contenção de novos casos. Demonstra-se por meio da análise da publicização dos boletins epidemiológicos da Covid-19, disseminados nas redes sociais do Governo do Estado de Alagoas, a importância da transparência nas informações de interesse público. Essas redes, principais meios de comunicação e informação da sociedade, têm alto poder de disseminação informacional, cujo potencial “viral”, proporcionado pela instantaneidade e (...) interatividade, alcança uma infinidade de públicos. Como espaços férteis para o surgimento de Fake News e desinformação, podem gerar crises de confiabilidade e veracidade quanto às informações divulgadas pelo Governo, bem como não estimular o alagoano ao exercício da cidadania, nem agregar valor a essa sociedade. A partir do exposto, constata-se que a informação, nesse ambiente digital, deve ser tratada de forma crível, clara e responsável, com vistas a gerar maior confiabilidade, interação e acesso à informação de interesse social, bem como contribuir para a redução da circulação de Fake News e Desinformação. Adotou-se as pesquisas documental e qualitativa com uma abordagem exploratória, realizando-se a coleta de dados nas redes sociais oficiais do governo alagoano: Facebook, Instagram e Twitter. Este trabalho tem por objetivo apresentar diferentes concepções sobre a educação das crianças na perspectiva de filósofos e educadores, considerados como alguns dos autores mais representativos no estudo do tema. Trata-se de uma pesquisa de caráter bibliográfico, fundamentada na construção de conhecimentos oriundos das contribuições de autores clássicos e contemporâneos, além de estudos posteriores feitos por estudiosos e pesquisadores sobre as idéias de tais autores. Esse procedimento é necessário porque nem todos os autores trataram sistematicamente do tema em questão, mas (...) apenas abordaram-no esporadicamente em aforismos, ensaios e proposições. Assim, teremos seqüencialmente o estudo e a interpretação de algumas idéias encontradas nas obras Platão, Montaigne, Descartes, Locke, Rousseau, Kant, Vygotsky, Dewey, Piaget e Lipman. A partir da compreensão dos avanços e retrocessos no estudo da educação da criança defendemos a necessidade e a relevância do estudo dessa temática no âmbito acadêmico, particularmente, na disciplina filosofia da educação, pois conforme acreditamos, a mesma constitui um dos principais campos teóricos na contribuição para a formação de futuros educadores. Objective: Symptom improvement is often examined as an indicator of a good outcome of accessing mental health services. However, there is little evidence of whether symptom improvement is associated with other indicators of a good outcome, such as a mutual agreement to end treatment. The aim of this study was to examine whether young people accessing mental health services who meaningfully improved were more likely to mutually agree to end treatment.Methods: Multilevel multinomial regression analysis controlling for age, gender, ethnicity, and (...) referral source was conducted on N = 8,995 episodes of care [Female = 5,469, 61%; meanAge = 13.66 years] using anonymised administrative data from young people's mental health services.Results: Compared to young people with no change in mental health difficulties, those showing positive meaningful changes in mental health difficulties were less likely to have case closure due to non-mutual agreement. Similarly, they were less likely to transfer or end treatment for other reasons than by case closure due to mutual agreement.Conclusion: The findings suggest that young people accessing mental health services whose symptoms meaningfully improve are more likely to mutually agree to end treatment, adding to the evidence that symptom improvement may be appropriate to examine as an indicator of a good outcome of accessing mental health services. Objective: This study aims to compare the acoustic vocal analysis results of a group of transgender women relative to those of cisgender women.Methods: Thirty transgender women between the ages of 19 and 52 years old participated in the study. The control group was composed of 31 cisgender women between the ages of 20 and 48 years old. A standardized questionnaire was administered to collect general patient data to better characterize the participants. The vowel /a/ sounds of all participants were collected (...) and analyzed by the Multi-Dimensional Voice Program advanced system.Results: Statistically significant differences between cisgender and transgender women were found on 14 measures: fundamental frequency, maximum fundamental frequency, minimum fundamental frequency, standard deviation of fundamental frequency, absolute jitter, percentage or relative jitter, fundamental frequency relative average perturbation, fundamental frequency perturbation quotient, smoothed fundamental frequency perturbation quotient, fundamental frequency variation, absolute shimmer, relative shimmer, voice turbulence index, and soft phonation index. The mean fundamental frequency value was 159.046 Hz for the cases and 192.435 Hz for the controls.Conclusion: Through glottal adaptations, the group of transgender women managed to feminize their voices, presenting voices that were less aperiodic and softer than those of cisgender women. Assim, a Teologia da Educação presente na religiosidade da igreja cristã elabora um fundamento educativo, com vistas a propagação e manutenção de sua profissão. A entrevista foi articulada durante a visita da antropóloga Clarice Novaes da Mota no mês de abril de 2018 ao Rio de Janeiro – RJ, sendo os diálogos articulados virtualmente durante os meses de maio e junho de 2018. A entrevista traz um dos primeiros trabalhos antropológicos com os Povos Indígenas do Nordeste, sendo este construído pela Clarice Novaes da Mota com os Xokó no Estado de Sergipe. O objetivo deste artigo é, primeiro, apresentar e considerar as críticas de Paley com mais detalhes e, em seguida, discutir algumas das aplicações significativas da fenomenologia que muitas vezes foram negligenciadas pelos pesquisadores qualitativos. Como foi amplamente demonstrado ao longo dos anos, a fenomenologia pode não apenas fazer a diferença no manuseio, análise e interpretação dos dados disponíveis, mas também em como os dados são obtidos em primeiro lugar, por exemplo, através de técnicas especiais de entrevista. Consideraremos algumas figuras centrais (...) da psicologia fenomenológica clássica e da psiquiatria fenomenológica e apresentaremos alguns dos desenvolvimentos mais recentes na ciência cognitiva. Em seguida, discutiremos três casos concretos que demonstram como a fenomenologia foi aplicada no trabalho clínico com pacientes com esquizofrenia, paralisia cerebral e negligência hemispatial. A percepção é a maneira como interagimos com o mundo exterior. É a nossa capacidade de receber informações sensoriais do ambiente, como ver, ouvir, tocar e cheirar. Através da percepção, experimentamos o mundo e seus objetos como eles são em sua materialidade. A imaginação, por outro lado, é a capacidade de criar representações mentais e de ir além das informações sensoriais diretas, nos permitindo conceber o que não está presente, recriar o passado, ou até mesmo antecipar o futuro. Baseados na (...) interpretação sartriana segundo a qual há uma tensão dinâmica entre percepção e imaginação, nosso objetivo é problematizar a distinção rígida entre esses dois conceitos, reconhecendo que essa dinâmica desempenha um papel central em nossa experiência existencial e que a relação entre esses dois elementos não é dicotômica, mas sim uma interação constante que enriquece nossa compreensão do mundo e de nossa própria existência. Accountability is a quality often demanded of the church and its leaders today, and especially so within the Roman Catholic Church. But how should accountability itself be understood, and how might a more accountable church be achieved? This essay explores these questions from a new angle by offering a detailed ethical analysis of how accountability operates within broad-based community organizing (BBCO), a form of democratic politics with a highly developed theory and practice of accountability in which many churches already participate. (...) In dialogue with BBCO, the essay develops a constructive framework for conceptualizing accountability. It makes a case for understanding accountability principally as a property of reciprocal relationships between persons, and stresses the need to proactively cultivate and sustain relationships of accountability through ongoing democratic practices of accountability. It also highlights the role played by moral authority, power, and virtue in these relationships and practices. The essay concludes by using this framework to propose a new interpretation of the Catholic Church’s accountability crisis. It suggests a more accountable church may only be achieved when the whole people of God begin organizing themselves to build it. (shrink)

This essay looks closely at metaphors and other figures of speech that often feature in how Americans talk about dementia, becoming part of cultural narratives: shared stories that convey ideas and values, and also worries and fears. It uses approaches from literary studies to analyze how cultural narratives about dementia may surface in conversations with family members or health care professionals. This essay also draws on research on a notable social effect of legalizing medical aid in dying: patients may find (...) it easier to bring up a range of concerns, regardless of whether they have any interest in hastening their own death. The essay proposes that health care professionals rethink an idea prominent within their own culture: that conversations about the end of a person's life are inherently difficult. This framing may make it hard for people facing dementia to bring up their concerns about what lies ahead. We suggest a different way to think about these conversations, using the metaphor of “opening the door” to represent inviting a patient to bring up issues of deepest concern. (shrink)

The objective of this paper is to make visible and reflect upon the lives and experience of children from the perspective of alterity, Contesting a traditional gaze that understands childhood from the perspective of lack and negativity. The researchers–a doctoral researcher and an early childhood educator– conducted the study with two groups of children aged between four and six years old, in early childhood educational spaces in two distinct and socially unequal contexts: a group of children living in a favela (...) and a middle/upper class group. Data was produced from observing the interactions of children in play situations, and from individual and collective interviews, photographs and photo exchanges. The researchers sought, through the theoretical lenses of philosophy, sociology, and geography of childhood, to reflect on how children signify their experiences and how they announce new possibilities of relating to school culture. Play is identified as an experience that unites them as a social group, regardless of the unequal conditions in which they live, and through which it is possible to create escape routes, transcend institutional limits, exercise otherness, and produce joyful interaction. Through the ways in which they exercise their relationships with with the world through play, spaces become places and territories of childhood, expressing assigned meanings that interrogate those conventionally instituted by adults; and time is lived by children from the perspective of experience and reiteration, which confronts the chronological, linear time of rigid and standardized routines. Finally, looking at childhood from the perspective of alterity provokes us to think and produce other modes of relationship between adults and children, beyond a colonizing and tutelary perspective, as an opening and reinvention. Since one of psychopaths’ most glaring deficits is a lack of empathy, and they are widely considered to be amoral, psychopaths are often taken as a test case for the hypothesis that empathy is necessary for moral agency. However, people with autism also lack empathy, so it is reasonable to think that (...) any empirically-informed attempt to answer the question of whether empathy is necessary for moral agency should give due attention to autism as well as psychopathy. Jeanette Kennett’s thought-provoking paper ‘Autism, Empathy and Moral Agency’ (2002) is an early and arguably the most notable example of such an attempt. Kennett argues that although autistic people and psychopaths both lack empathy, autistic people possess a ‘reverence for reason’ that enables them to become capable moral agents. By contrast, psychopaths lack this rational capacity, and it is this defect of reason rather than an empathic impairment that explains why they are amoral. Kennett thus concludes that empathy is not necessary for moral agency. In this chapter, I challenge Kennett’s argument. First, I review the empirical evidence in order to demonstrate that there is a component of empathy called affective empathy that is impaired in psychopaths but largely preserved in autistics. As such, the claim that psychopaths and autistics share a common lack of empathy is unjustified. Second, I challenge Kennett’s claim that empathy plays no role in explaining the moral difference between psychopaths and autistics. Instead, I contend that the intact affective empathy of autistic people is a crucial component of their capacity to act from reverence for reason. (shrink)

On 19 July 2016, three medical organisations filed a federal lawsuit against representatives from several Vermont agencies over the Patient Choice and Control at End of Life Act. The law is similar to aid-in-dying laws in four other US states, but the lawsuit hinges on a distinctive aspect of Vermont's law pertaining to patients' rights to information. The lawsuit raises questions about whether, and under what circumstances, there is an ethical obligation to inform terminally ill patients about AID as an (...) end-of-life option. Much of the literature on clinical communication about AID addresses how physicians should respond to patient requests for assisted dying, but neglects the question of how physicians should approach patients who may not know enough about AID to request it. In this article, I examine the possibility of an affirmative duty to inform terminally ill patients about AID in light of ethical concerns about professional responsibilities to patients and the maintenance of the patient–provider relationship. I suggest that we should not take for granted that communication about AID ought to be patient-initiated, and that there may be circumstances in which physicians have good reasons to introduce the topic themselves. By identifying ethical considerations that ought to inform such discussions, I aim to set an agenda for future bioethical research that adopts a broader perspective on clinical communication about AID. Stresses on healthcare systems and moral distress among clinicians are urgent, intertwined bioethical problems in contemporary healthcare. Yet conceptualizations of moral distress in bioethical inquiry often overlook a range of routine threats to professional integrity in healthcare work. Using examples from our research on frontline physicians working during the COVID-19 pandemic, this article clarifies conceptual distinctions between moral distress, moral injury, and moral stress and illustrates how these concepts operate together in healthcare work. Drawing from the philosophy of healthcare, we (...) explain how moral stress results from the normal operations of overstressed systems; unlike moral distress and moral injury, it may not involve a sense of powerlessness concerning patient care. The analysis of moral stress directs attention beyond the individual, to stress-generating systemic factors. We conclude by reflecting on how and why this conceptual clarity matters for improving clinicians’ professional wellbeing, and offer preliminary pathways for intervention. (shrink)

“It's almost like putting salt in a wound, for this person who's already made a very difficult decision,” suggested Meghan Patterson, a licensed obstetrician-gynecologist whom we interviewed in our qualitative study of the experiences of North Carolina abortion providers practicing under the state's Woman's Right to Know Act. The act requires that women receive counseling with state-mandated information at least twenty-four hours prior to obtaining an abortion. After the law was passed, Patterson worked with clinic administrators, in consultation with a (...) lawyer, to write a script to be used in the state-mandated counseling procedure. She and her colleagues took particular steps to mitigate the effects of what she described as HB 854's “forced language”—such as referring to the “father of the child.” While HB 854 stipulated that patients must be informed of the medical risks associated with the particular abortion procedure as well as those of carrying the child to term, Patterson's script made explicit the magnitude of comparative risks, emphasizing that the risks of carrying a pregnancy to term are substantially greater than the risks of an early-term abortion. She felt that these contextualization strategies helped to facilitate trust and rapport in a clinical care situation that proved relationally and morally challenging. In this article, we take up and expand on this point by elucidating an empirically grounded approach to ethically justified care when health care providers face legal or institutional policy mandates that raise possible moral conflicts. Our approach builds on recent bioethics discourse addressing conscience in the practice of medicine. While the concept of conscience has broad philosophical underpinnings relating to moral judgment, agency, and discernments of right and wrong, debates in bioethics have tended to engage the concept primarily vis-à-vis rights of conscientious objection or refusal. Here, we suggest a broader frame for thinking about claims of conscience in health care. Our approach draws on the feminist bioethics and the ethics of care literatures to highlight how providers may be motivated by matters of conscience, including relational concerns, in the active provision of certain forms of care. What emerges are two possibilities: not only conscientious refusal to comply with a policy mandate but also conscientious compliance—working conscientiously within a mandate's confines. (shrink)

Background Bioethical debates about privacy, big data, and public health surveillance have not sufficiently engaged the perspectives of those being surveilled. The data justice framework suggests that big data applications have the potential to create disproportionate harm for socially marginalized groups. Using examples from our research on HIV surveillance for individuals incarcerated in jails, we analyze ethical issues in deploying big data in public health surveillance. -/- Methods We conducted qualitative, semi-structured interviews with 24 people living with HIV who had (...) been previously incarcerated in county jails about their perspectives on and experiences with HIV surveillance, as part of a larger study to characterize ethical considerations in leveraging big data techniques to enhance continuity of care for incarcerated people living with HIV. -/- Results Most participants expressed support for the state health department tracking HIV testing results and viral load data. Several viewed HIV surveillance as a violation of privacy, and several had actively avoided contact from state public health outreach workers. Participants were most likely to express reservations about surveillance when they viewed the state’s motives as self-interested. Perspectives highlight the mistrust that structurally vulnerable people may have in the state’s capacity to act as an agent of welfare. Findings suggest that adopting a nuanced, context-sensitive view on surveillance is essential. -/- Conclusions Establishing trustworthiness through interpersonal interactions with public health personnel is important to reversing historical legacies of harm to racial minorities and structurally vulnerable groups. Empowering stakeholders to participate in the design and implementation of data infrastructure and governance is critical for advancing a data justice agenda, and can offset privacy concerns. The next steps in advancing the data justice framework in public health surveillance will be to innovate ways to represent the voices of structurally vulnerable groups in the design and governance of big data initiatives. The global crisis of the COVID-19 pandemic has considerably accelerated the adoption of teleconsultation—a form of consultation between patient and health care professional that occurs via videoconferencing platforms. For this reason, it is important to investigate the way in which this form of interaction modifies the nature of the clinical encounter and the extent to which this modification impacts the healing process. For this purpose, I will refer to insights into the clinical encounter as a face-to-face encounter drawn from the (...) phenomenology of medicine (R. Zaner, K. Toombs, E. Pellegrino). I will also take into account a criticism that has been expressed by various contemporary phenomenologists (H. Dreyfus, T. Fuchs, L. Dolezal, H. Carel), namely, that due to the lack of physical proximity to the other in all types of online encounters, such encounters lack significant features that are present in face-to-face encounters, with the most important of these being the possibility of attaining an empathetic perception of the other and a sense of embodied risk. As these elements are essential features of the clinical encounter, the aim of this paper is to determine whether teleconsultation exhibits these features. To do that, I will integrate phenomenological philosophy with qualitative research drawing materials from both the philosophical tradition, particularly with respect to the concepts of the face-to-face encounter and embodied risk (A. Schutz and H. Dreyfus), and qualitative research study regarding patient experiences of teleconsultation. I will argue that teleconsultation does involve both the possibility of perceiving the other empathetically and the possibility of experiencing a sense of embodied risk. (shrink)

The hoax perpetrated by New York University theoretical physicist Alan Sokal in 1996 on the editors of the journal Social Text quickly became widely known and hotly debated. (See Physics Today January 1997, page 61, and March 1997, page 73.) "Transgressing the Boundaries -- Toward a Transformative Hermeneutics of Quantum Gravity," was the title of the parody he slipped past the unsuspecting editors. [1] Many readers of Sokal's article characterized it as an ingenious exposure of the decline of the intellectual (...) standards in contemporary academia, and as a brilliant parody of the postmodern nonsense rampant among the cultural studies of science. Sokal's paper is variously, so we read, "a hilarious compilation of pomo gibberish", "an imitation of academic babble", and even "a transformative hermeneutics of total bullshit".
